Position: Lead Mental Care Therapist (LCSW) & Clinical Mental Health Lead (PMHNP) – Home Health & Wellness

Location: Hallandale Beach / Broward County, FL | Hybrid / In-Home Care

Job Type: 1099 Contractor or W-2 (negotiable)

We’re now launching our Mental Wellness Division, and we’re hiring both a Licensed Clinical Social Worker (LCSW) and a Psychiatric Mental Health Nurse Practitioner (PMHNP) to shape and lead our emotional and cognitive support offerings. Psychiatric Mental Health Nurse Practitioner (PMHNP)

Clinical Lead – Mental Wellness Division

Responsibilities:

• Provide 1:1 therapy, medication management, and group sessions
• Help develop Surve’s mental wellness programs and SOPs
• Maintain Medicare-compliant documentation
• Lead initiatives in emotional health, mindfulness, and cognitive care
• Collaborate with LCSW, Director of Nursing, and full care team
• Drive clinical vision and strategic growth of the division
• Serve as the clinical point of contact for mental health services within the organization
• Participate in client care planning, assessments, and progress reviews.

Requirements/ Qualifications :

• Active Florida PMHNP license ( Required )
• Medicare-eligible ( Required )
• 2+ years experience working with adults
• Therapy + psychopharmacology experience
• Strong systems thinking and independent work ethic
• Comfortable working independently and helping build systems from the ground up
• Strong interpersonal, documentation, and care coordination skills
• Concierge, private-pay, or wellness industry experience a plus

Licensed Clinical Social Worker (LCSW)

Lead Therapist & Intern Supervisor

Responsibilities:

• Provide individual and group therapy to private-pay and Medicare clients (issues: grief, anxiety, depression, trauma, stress, transitions, relational challenges)
• Lead support groups, workshops, and wellness programs
• Maintain Medicare-compliant documentation
• Collaborate with our nursing and wellness team to align care plans
• Serve as a Qualified Supervisor for LCSW interns (RCSWIs)
• Help recruit, train, and mentor future therapists
• Participate in client care planning, assessments, and progress reviews

Requirements/ Qualifications :

• Active Florida LCSW license ( Required )
• Medicare-eligible ( Required )
• Qualified Supervisor status or willingness to obtain ( Required** for intern supervision)
• 2+ years of experience with adults
• Strong documentation, collaboration, and leadership skills
• Group therapy experience strongly preferred
• Comfortable working independently and helping build systems from the ground up
• Strong interpersonal, documentation, and care coordination skills
• Concierge, private-pay, or wellness industry experience a plus
